Essential collaboration between local healthcare professionals

We believe in the importance of regional networks to strengthen coordination of health services between the community and the hospital, optimize care, and improve patients’ quality of life. Our goal is to work collaboratively with the various stakeholders in the care continuum—including community health centers and private practices—to develop personalized care plans tailored to each patient’s specific needs and circumstances.
